Community Oriented Approach to Coordinated Healthcare - Managing Chronic Disease

Topsail developed this system in collaboration with the Duke Division of Community Health to support the information needs of providers serving Medicaid beneficiaries in the Durham County Carolina Access II program. COACH connects over 30 Primary Care Clinics, 3 Urgent Care Facilities, 5 Emergency Departments, 5 Hospitals, and 5 Governmental Agencies that all rely on its information to effectively manage their patients' chronic illnesses. The COACH database manages 185,000 patients, has over 14 million patient encounter records, and supports over 400 users. Also, did we mention that COACH was one of Topsail’s first projects and it is still operation after 24 years!
